Meniscus Tear Treatment: Do You Really Need Surgery?

A torn meniscus is one of the most common knee injuries we see, and one of the most over-treated. The right meniscus tear treatment depends on the type of tear, your age, your activity goals, and what your knee actually needs, not on a rush to the operating room. What a Meniscus Tear Actually Is

Each knee has two menisci, C-shaped pads of cartilage that cushion the joint between your thigh bone and shin bone. When one tears, you may feel joint-line pain, swelling, stiffness, clicking, or a sense that the knee catches or gives way.

Tears generally fall into two broad groups:

  • Acute (traumatic) tears, often from a twist or pivot during sports, more common in younger, active knees.

  • Degenerative tears, which develop gradually as the cartilage wears with age and may show up on imaging even without a clear injury.

The pattern matters too. Bucket-handle, radial, horizontal, flap, and small frayed tears all behave differently, which is why imaging alone never tells the whole story.

Why Many Tears Do Not Need Surgery

Here is the part most people are not told: a large share of meniscus tears, especially degenerative ones, do well without surgery. The cartilage has a limited blood supply, so some tears will not heal with stitches anyway, while others calm down once the surrounding muscles and mechanics improve.

Good evidence shows that for many degenerative tears, structured physical therapy produces results comparable to arthroscopy over time, without the recovery and cost of an operation. A tear seen on MRI is not automatically the source of your pain, and plenty of people have meniscus changes on imaging with no symptoms at all. The honest question is not "is there a tear" but "is this tear the problem, and will surgery actually fix it."

Conservative Meniscus Tear Treatment Options

When surgery is not the right first move, an effective non-surgical plan usually combines several tools:

  • Activity modification to unload the knee while it settles, without shutting down your whole routine.

  • Physical therapy to restore strength, mobility, and control. Our integrated PT with Dr. Christie Booth Smith, PT, DPT, KEOMPT, builds a plan around your specific tear and goals.

  • Bracing or DME when added support helps you stay active during recovery.

  • Injections in selected cases, including hyaluronic acid for lubrication, or orthobiologics such as PRP and BMAC where appropriate.

  • Shockwave therapy (StemWave) for certain stubborn soft-tissue and joint-related symptoms.

Many patients turn a corner with this approach and never need an operation. For transparent pricing, a physical therapy initial evaluation and treatment (60 minutes) is 235 dollars, follow-up PT sessions are 195 dollars, and an evaluation plus five sessions is 1,000 dollars. A StemWave consultation is 50 dollars and a standalone StemWave treatment is 200 dollars. For biologics and hyaluronic acid, ask whether you are a candidate and request a Good Faith Estimate.

When Arthroscopy Is Actually Warranted

Surgery earns its place in specific situations. Arthroscopic meniscus treatment, whether a repair or a trim of an unstable fragment, is worth serious consideration when:

  • A knee locks or truly cannot fully straighten, often from a displaced bucket-handle tear.

  • A repairable tear occurs in a younger, active patient where saving the meniscus protects the knee long term.

  • Real mechanical symptoms (catching, locking, giving way) persist despite a fair trial of conservative care.

  • An acute, high-grade tear sits alongside another injury, such as an ACL tear, that already calls for surgery.

When repair is possible, we favor preserving meniscus tissue, because the cartilage you keep protects your knee for decades.
